Your Rights as a Consumer
1. Receive care in a safe, respectful, and professional environment.
2. Be treated with dignity, courtesy, and privacy at all times.
3. Participate in planning your care and to refuse or request changes to your services.
4. Receive a clear explanation of services, schedules, and costs before care begins.
5. Be informed of the names and qualifications of caregivers providing services.
6. Request replacement of a caregiver if you are dissatisfied with performance or compatibility.
7. Receive care without discrimination based on age, race, color, religion, sex, national origin, disability, or source of payment.
8. Have your personal information kept confidential in accordance with state and federal privacy laws.
9. Be informed of how to file a complaint or grievance without fear of reprisal.
10. Expect courteous and timely communication from our staff.
Your Responsibilities
To help us provide the best possible home health care, clients or their representatives are asked to:
• Provide accurate health and service information.
• Notify Green Oaks of any changes in condition, address, or emergency contacts.
• Treat caregivers with courtesy and respect.
• Provide a safe environment for care delivery.
• Notify Green Oaks in advance if services need to be canceled or rescheduled.
